If you've ever seen ESR or CRP on a lab report and wondered what they measure, here's the plain-language answer: they measure inflammation — your body's alarm response to infection, injury or chronic disease.
What Is Inflammation?
When something is wrong — an infection, tissue damage, an autoimmune flare — your immune system releases chemical signals. ESR and CRP rise in response, making them useful "smoke detectors". They don't say *where* the fire is, but they confirm something is burning.
ESR (Erythrocyte Sedimentation Rate)
Measures how fast red blood cells settle in a tube over one hour. Inflammation proteins make cells clump and settle faster.
- Rises slowly (days) and falls slowly
- Useful for tracking chronic conditions — arthritis, tuberculosis, chronic infections
- Mildly affected by age, anaemia and pregnancy, so interpretation needs context
CRP (C-Reactive Protein)
A protein made by the liver within hours of inflammation starting.
- Rises fast (6–12 hours) and falls fast once the cause resolves
- Better for acute infections and monitoring treatment response
- hs-CRP (high-sensitivity version) at persistently mild elevation is also a recognised marker of cardiovascular risk
Common Causes of Elevation
- Infections — bacterial infections push CRP especially high
- Autoimmune diseases — rheumatoid arthritis, lupus
- Recent injury or surgery
- Obesity and metabolic syndrome (chronic low-grade inflammation)
- Smoking
